LegitScript Certification and Google Ads Policy for Addiction Treatment
By Shane Burke, Clicktuitive·Last updated August 2026
Every US provider advertising addiction treatment services needs LegitScript certification plus Google's own verification before a single ad can run. Here's how the process actually works, including the costs, the rejection traps, and what happens after approval.
In 2017, Google suspended addiction treatment advertising almost entirely after widespread abuse by lead brokers and patient brokering operations. Advertising came back in 2018 with a gate. Advertisers must first be certified by LegitScript, a third party verification company, and then separately approved by Google. The gate is real, and it's also a moat. Providers who complete certification compete in a much smaller field than in any other healthcare vertical.

Lead generators and call centers. Referral platforms. Sober living homes without licensed clinical services. And one that surprises people: any provider affiliated with lead generators. That affiliation alone is an immediate denial.

There's no official standard timeline. LegitScript declines to estimate one, because review speed depends on how complete your application is and how quickly you respond to follow-up requests. Plan for several weeks of calendar time and treat every document request as same-day urgent.
The facility address is buried or missing from the website.
Service descriptions overreach the license, like advertising detox when you're licensed for residential only.
Stock photos presented as your facility.
Expired licenses or insurance in the application packet.
Missing written policies.
Any relationship with a lead generator.
Patient incentives, since paying for travel, rent, or insurance premiums is disqualifying.
Disciplinary history within the 10 year lookback period.
After LegitScript certifies you, you still have to complete Google's own advertiser verification for addiction services, tied to your Google Ads account. Both must clear before ads serve. This is where accounts stall most often. The certification gets done and then nobody completes the Google side, or the ad account's business details don't match the certified entity. It's also where an experienced agency earns its fee, because we've run this handoff enough times to know what Google's reviewers flag. One more thing worth knowing: the same certification is recognized by Meta, Microsoft Ads, and Nextdoor, so one approval opens four ad platforms.
LegitScript monitors certified advertisers continuously. Website changes that drift from your licensure, acquired domains you never disclosed, or ad copy that violates policy can all put certification at risk, and a suspended certification pauses every ad platform at once. We review client sites and ad copy against certification standards as part of ongoing management, because the fastest way to lose your census pipeline is to lose your ability to advertise.
At published rates, providers with 1 to 9 facilities pay a $1,595 nonrefundable application fee plus a $3,095 annual certification fee per facility. Multi-facility and individual practitioner tiers are priced lower. Expedited review costs an additional $2,500.
LegitScript publishes no official timeline. Complete applications with fast document turnaround move much quicker than incomplete ones, so preparation is the biggest lever you control.
Not unless it offers licensed clinical services. Sober living without clinical licensure isn't eligible for LegitScript addiction treatment certification, and Google requires that certification.
Yes. Meta, Microsoft Ads, and Nextdoor all recognize the same certification for addiction treatment advertising.
Most often the Google-side advertiser verification was never completed, or the ad account's business details don't match the certified entity. Both layers have to clear before ads serve.
Certification gets you into the auction. It doesn't make the auction efficient. If you're certified and spending, the Free Account Scan will show you where that spend is leaking and what your cost per admit actually is.
